29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   On or about August 16, 2017, employees were working on the construction of a residential home. Two employees were observed working on the framework without any form of fall protection. Employees were exposed to fall hazards of more than six feet to the ground below.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill:  On or about August 16, 2017,  the employer did not ensure his employees were retrained in any inadequacies that were indicated in their knowledge or use of fall protection systems. The employees were exposed to fall hazards of greater than six feet to the ground below.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:  On or about August 16, 2017, employees were working on the construction of a residential home. One employee was observed working off an A-Frame step ladder and was straddling it while sitting on the top step. The employee was exposed to fall hazards.
