29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    On or about 8/16/2017, the employer failed to ensure that the guarding on machinery including, but not limited to the Blister machine, RBS Equipment Designs Limited, Model TBS, Serial # 20-122, had adequate guarding. The opening between the board pins, where the product sits, and the guard was approximately 1-5/8". The distance to the point of operation was approximately 3-5/8". The guard should have been adjusted to 5/8" from the top of the board pins.

29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):  On or about 8/16/2017, the employer failed to ensure that the forklift training that was conducted approximately 2-1/2 years ago, included a certification of each forklift operator. The employees operating the Toyota 7FBCU25 forklift, serial #61375 did not have documentation/certification of the training received.
