29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and on an incident report (OSHA Form 301 or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness had occurred:  a) Norfolk Naval Shipyard, St. Juliens Creek Annex Building 181, Portsmouth, VA 23709: On or about April 25, 2017, and times prior thereto, the employer did not enter an injury that occurred on March 16, 2017, on the OSHA Form 300 or equivalent and on an incident report within 7 calendar days.  Abatement documentation is required for this item.

29 CFR 1904.32(b)(2)(ii): The annual summary did not include one or more of the following elements: the calendar year covered, the company's name, establishment name, establishment address, annual average number of employees covered by the OSHA 300 Log, and the total hours worked by all employees covered by the OSHA 300 Log.    a) Norfolk Naval Shipyard, St. Juliens Creek Annex Building 181, Portsmouth, VA 23709: The employer combined the recordable injuries and illnesses, number of employees and total hours worked from a Charleston, SC location with the establishment at Norfolk Naval Shipyard on the OSHA 300A Summary log or equivalent form.    Abatement documentation is required for this item.
