29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:  (a) Apartment building 25, Ladder jack scaffold platform located outside Unit #2527: On or about 08/15/2017, and at times prior, the employer exposed an employee to amputation hazards while operating a DeWalt grinder, in that the guard this tool was manufactured with was not used while cutting Hardie-Plank Siding.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  (a) The Nesbit Palisades Apartment Home, Building 25 outside unit #2527: On or about 08/15/2017, and at times prior, the employer exposed employees to fall hazards of 12-feet, 2-inches to the lower level, in that a complete fall protection system was not provided to employees installing windows from a ladder jack scaffold platform.
