29 CFR 1910.272(g)(1)(ii): Prior to entry, all mechanical, electrical, hydraulic, and pneumatic equipment which presented a danger to employees inside grain storage structures in the grain handling facility were not deenergized, disconnected, locked out and tagged, blocked-off, or otherwise prevented from operating by other equally effective means or methods.  During the annual cleaning in May 2017, the employer did not require machine lock out inside the storage bin while employees cleaned out the corn. Failure to de-energize and lock out equipment exposes employees to the hazards of being struck by equipment such as, but not limited to the sweep auger.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about August 18, 2017, and at times prior thereto, the employer did not establish an energy control program to include training and periodic inspections, exposing employees to the hazard of being struck by or caught in machines and/or equipment.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  On or about August 18, 2017, the employer was using an industrial fan where the end of the cord was not equipped with strain relief and the insulated wires were exposed, exposing the employee to an electrical shock hazard.
