29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:    On or about August 16, 2017, at 2408 Misty Ivy Court, Buford, Georgia:    Two employees performing roofing work were exposed to fall hazards of approximately 21-feet while using an aluminum extension ladder with a damaged rung, at an angle with the base not set at one-fourth of the working height of the ladder to the top support when the employer did not inspect the jobsite.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about August 16, 2017, at 2408 Misty Ivy Court, Buford, Georgia:    Two employees performing roofing work on an approximately 8/10 slope roof were exposed to a fall hazard of approximately 21-feet when they worked on the roof without using fall protection.
