29 CFR 1926.501(b)(13):   "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems.  On 8/17/2017, employees from Matt Rowles Construction, Inc. were sheathing the roof of a residential building located at lot 88 on Brookfield Crossing Drive in Wentzville, MO. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.   The foreman and two employees were observed on 7/12 and 10/12 sloped roofs approximately nine feet above the ground without fall protection, exposing them to a fall to the ground below.

29 CFR 1926.1053(b)(1):   When portable ladders were used for access to the upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.  On 8/17/2017, employees from Matt Rowles Construction, Inc. were sheathing the roof of a residential building located at lot 88 on Brookfield Crossing Drive in Wentzville, MO.  The ladder the employees were using to access the roof did not extend three feet beyond the upper landing surface or edge of the roof.
