29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a)  On or about August 18, 2017, employees were exposed to falls hazards while roofing a house without properly using fall protection.  The fall hazard ranged from 11 feet from the lower level roofs, to 20 feet and greater on the upper level roofs. 29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:  (a)  On or about August 18, 2017, employees were exposed to fall hazards while reroofing a building when multiple workers were attached to an anchor that was designed for only one person.  Three different one-person anchors had two workers attached to them on building 14 of the Dublin Townhomes.  Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  On or about August 18, 2017, employees exposed to fall hazards and had not been trained by the employer to recognize and prevent the hazards.  Employees had personal fall arrest systems that were provided by the employer, but they did not use them properly; harnesses were worn too loosely, some workers did not use ropes, some did not attach ropes to anchors, and some had multiple ropes attached to one anchor.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
