29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) On or about 8/16/2017 - at 870 Vestalake Ridge Drive, Vestavia, AL, employees were exposed to fall hazards of approximately 21 feet 6 inches above ground level while installing shingles on a roof with a 12:12 pitch.

29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  (a) On or about 8/16/2017 - at 870 Vestalake Ridge Drive, Vestavia, AL, employees were exposed to fall hazards while improperly using a body positioning belt with the lanyard attachment point located at the wearer's waist.  The employees were on a 12:12 pitched roof with a fall exposure of approximately 18 feet 4 inches from the eave to the lower level.
