29 CFR 1910.28(b)(6)(i): (i) Each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard.    a. Honda Transmission Manufacturing of America, Inc. located in Russells Point, Ohio:  On or about August 11, 2017, the employer did not assure the working surface on top of the dip well for furnace 1652 was free of hazards.  An employee changing out a dip well filter stepped through a 6 inch wide opening and fell through into the 1200 degree F molten aluminum bath.    No additional abatement information is needed for this item.

29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    a. Honda Transmission Manufacturing of America, Inc. located in Russells Point, Ohio:  On or about August 11, 2017, the employer did not ensure employees exposed to molten aluminum splashes when performing operations such as but not limited to filter changes in the molten metal furnace dip wells, were provided with flame retardant clothing for the legs.
