29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    (a) Zayo Group at 10475 East 112th Avenue, Henderson, CO 80640:  On or before August 22, 2017 the exposing employer did not ensure that employees working in forklifts were trained to operate the lift. The employer did not train and evaluate each operator who is authorized to operate the GW-GEHL RS8-42 forklift to ensure they were competent to operate the forklift safely. This exposed employees to potential struck by and struck against hazards.

(a) Zayo Group at 10475 East 112th, Henderson, CO 80640:  On and before August 22, 2017, the employer did not maintain a safety data sheet for each hazardous chemical and lubricants, flammable liquids, and combustible liquids stored in the warehouse. These chemicals include motor oil, coolant, antifreeze, gasoline, diesel, and cleaning supplies. This exposes employees to chemical irritations to eyes, respiratory, and skin due to employee lack of knowledge regarding chemical hazards.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  (a) Zayo Group at 10475 East 112th, Henderson, CO 80640: On or before August 22, 2017, the employer did not provide information and training to employees in regards to the chemicals use on-site and on the safety data sheets. Employees use chemicals such as motor oil, coolant, anti freeze, gasoline, diesel, and cleaning chemicals. 29 CFR 1910.37(a)(1): Exit routes must be kept free of explosive or highly flammable furnishings or other decorations.  (a) Zayo Group at 10475 East 112th, Henderson, CO 80640: On and before August 22, 2017, the employer did not ensure that emergency exits were kept free of flammable chemicals. Gasoline and Diesel were being stored in front of the emergency exit at the facility. This condition exposed the employees to the hazards associated with fire and smoke.
