OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.   Well site:  On or about August 23, 2017, the employer did not ensure the derrick or mast was provided with an auxiliary means of escape (Geronimo Line) and installed prior to allowing employees to work in the derrick. An employee was exposed to the hazard of fall from heights in the event of a fire or explosion.
