29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about August 23, 2017, employees were exposed to being struck by hazards.  The employees were installing roofing materials on a residential home located at 2296 Canterbury Farms, Hinckley, Ohio 44233.  There was an employee working on the ground underneath the upper employees, without wearing a protective helmet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about August 23, 2017, employees were exposed to a fall of approximately 15 feet.  The employees were installing roofing materials on a residential home located at 2296 Canterbury Farms, Hinckley, Ohio 44233.  The employees were working on all areas of the roof without using any form of fall protection.
