Employees were using safety glasses in lieu of splash goggles/facemask when adding water to a forklift battery.

This most recently occurred at 615 East 9th St., South Sioux City, Nebraska 68776 on July 13, 2017.  Employees were using cotton gloves for hand protection in lieu of acid-resistant gloves when adding water to a forklift battery.

The employer failed to ensure that an employee was not allowed to operate a lift truck daily before being retrained and their performance being evaluated following an incident where an employee was injured while operating a Nissan lift truck.
