You must enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.      a)  On August 6, 2017 a recordable injury occurred when an employees hand and arm was injured while scraping chicken manure and was not entered or recorded on the OSHA 300 log within 7 business days.

29 CFR 1910.134(c)(2): The employer did not develop and implement a respiratory protection program in accordance with sections i and ii of this paragraph (c)(2) for employees who chose to wear respirators where respirator use was not required:    a) The employer did not develop and implement the elements of a respiratory protection program to ensure that employees voluntarily wearing a supply air hooded respirator was medically able to use it and that they are cleaned, stored and maintained so that it does not present a health hazard to the wearer.     b) Employees voluntarily wearing 2-strap filtering facepiece (dust masks) and supply air hooded respirators were provided with the information contained in Appendix D of this standard.
