29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.   On or about August 24, 2017, at the north side of the commercial building, the employer did not ensure that employees working from an JLG man-lift were in use of fall protection, exposing the employees to a fall hazard.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.   On or about August 24, 2017, at the north side of the commercial building, the employer did not ensure an employee working below an elevated JLG man-lift was in use of a protective helmet, exposing the employee to struck-by hazards from falling tools and materials.
