29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph.Note: This paragraph does not apply to large area scaffolds where the employer has satisfied the following criteria: the ladder was placed against a structure which was not a part of the scaffold and the scaffold was secured against the sideways thrust On or about August 22, 2017: a) An employee installing metal trim work on a new residential structure was exposed to a fall hazard of approximately 20.5 feet while using an extension ladder on the scaffold to reach working area. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. On or about August 22, 2017: a) An employee installing metal trim on a new residential structure was exposed to a fall hazard of approximately 20.5 feet when the employer failed to ensure that the employee was using proper fall protection measures. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area. On or about August 22, 2017: a) An employee who was exposed to a fall hazard of approximately 20.5 feet while installing metal trim work, did not receive the necessary training from his employer to recognize, reduce and eliminate the fall hazard exposed to. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
