29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  Box-On-Demand, Line No. 2 - On or about August 11, 2017, employees were required to remove a jam of the tape and change the tape on the 'L' Clipper Tape machine. The written energy control did not include equipment specific step-by-step lockout procedures for the 'L' Clipper Tape machine. Employees were thereby exposed to the hazards of inadvertent start-up of the equipment.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:  Shipping Department - On or about August 17, 2017, employees required to remove a jam of the tape and change the tape on the 'L' Clipper Tape machine were not provided with locks for isolating, securing or blocking of machines or equipment from energy sources. Employee(s) were thereby exposed to the hazards associated with inadvertent start-up of equipment.
