29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Box-On-Demand, Line No. 2 - On or about August 11, 2017, employees were required to remove a jam of the tape and change the tape on the 'L' Clipper Tape machine. The written energy control did not include equipment specific step-by-step lockout procedures for the 'L' Clipper Tape machine. Employees were thereby exposed to the hazards of inadvertent start-up of the equipment.
