29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    On or about 08/23/2017, on the work site located at 4501 Euphrosine Street in Westwego, LA, the employer did not ensure that employees engaged in moving a hopper via crane were wearing hard hats to protect from the impact of overhead hazards.  An employee rigging the load and two others assisting from the ground were working beneath rigging materials/chains and/or the boom of the crane.

29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions:    On or about 08/23/2017, on the work site located at 4501 Euphrosine Street in Westwego, LA, the employer did not ensure that employees working in the street to move a hopper were wearing reflective clothing.  The crane and hopper blocked one lane of a two lane road such that through traffic used the other lane for both directions of travel.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or about 08/23/2017, on the work site located at 4501 Euphrosine Street in Westwego, LA, the employer did not ensure that employees engaged in moving a hopper were protected from falling from the top of the hopper onto the concrete below.  Employees were standing on top of the hopper at an elevation of approximately 12'.
