The workers were exposed to a struck-by hazard.

29 CFR 1926.502(d): Body belts were used after January 1, 1998 as part of personal fall arrest systems: JOB SITE: 109 Christopher Place, Saint Louis, MO 63129, on or about 8/18/2017, workers were observed utilizing body belts as part of their personal fall arrest system while on top of a 10:12 pitch roof, approximately 13' 6", actively installing new roofing materials.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: JOB SITE: 109 Christopher Place, Saint Louis, MO 63129, on or about 8/18/2017, workers were observed using a Werner Aluminum extension ladder, Model: D1824-2EQ, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. The workers were exposed to a fall hazard.
