29 CFR 1926.102(a)(1): 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury:  JOB SITE 109 Christopher Place, Saint Louis, MO 63129, on or about 8/18/17, workers were on top of the roof, actively install new roofing material operating pneumatic nail gun without any form of eye or face protection.  The workers were exposed to a struck-by hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  JOB SITE: 109 Christopher Place, Saint Louis, MO 63129, on or about 8/18/2017, workers were observed using a Werner Aluminum extension ladder, Model: D1824-2EQ, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  The workers were exposed to a fall hazard.
