The employees were exposed to a fall of 8'9".        Location: roof  On or about: 8/23/17    b) Employees, installing roof sheathing, were not provided with adequate fall protection. The employees were exposed to a fall of 27'.       NOTE: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:    Location: lower level, side entry  On or about: 8/23/17    a) Employees used a staircase for access between the lower level and first floor. The stairway, with 9 risers, did not have a handrail.     NOTE: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
