On or about August 24, 201, during the installation of roofing material at the job site located at 15586 Bennett Rd, North Royalton, Ohio 44133; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.
