29 CFR 1926.1053(b)(1): Where a portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a)  Worksite at 1422 1/2 Lee Street in Charleston, WV:  On or about August 25, 2017 employees were accessing the roof of a garage apartment from a deck using only one section of a 24 foot extension ladder that was not long enough to extend at least three feet above the roof edge.  The ladder was not secured at the top to a rigid support and a grasping device was not provided to assist in mounting and dismounting the ladder.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways:  (a)  Worksite at 1422 1/2 Lee Street in Charleston, WV:  On or about August 25, 2017 employees had not been trained by a competent person on the correct method of placing the ladder used to access the roof in that the ladder was not secured at the top to a rigid support and/or a grasping device was not provided to assist in mounting and dismounting the ladder when the ladder was not long enough to extend at least three feet above the roof edge.
