On or about August 26, 2017 at 50-22 Overbrook Street, Little Neck, NY 11362 Employees were observed working on a tubular welded scaffold approximately thirty (30) feet above the lower level installing bricks to a residential chimney without fall protection exposing employees to fall hazards. ABATEMENT CERTIFICATION IS NOT REQUIRED
