The path to ground from circuits, equipment, and enclosures was not permanent and continuous:  Location:  1721 Engle Dr., Arnold MO  63010  Employee operating Skil Saw, Model #HD77, with a missing ground pin.  The saw was used  to cut floor joist and band boards.

29 CFR 1926.405(a)(2)(ii): General requirements for temporary wiring - Extension cord sets used with portable electric tools and appliances were not of the three-wire type and were not designed for hard or extra-hard usage:  Location:  1721 Engle Dr., Arnold, MO  63010  On or about 08/25/2017, employees were using an approximately 250' Romax extension cord with a temp power pole.  The cord was being used to provide power for portable electric tools such as, but not limited to, Skil Saw, Model HD77.
