29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets:  a. On or about 08/24/2017, at the job site - employees conducting roofing work from a low slope roof were exposed to a 12 foot fall hazard, in that, fall protection was not used.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. On or about 08/24/2017, at the job site - the ladder used to access the roof did not extend at least 3 foot above the upper landing surface, exposing the employees to a 12 feet fall hazard.
