29 CFR 1910.67(c)(2)(v): A body belt was not worn and lanyard was not attached to the boom or basket when working from an aerial lift:   On or about August 30, 2017, at the above addressed site, an employee was exposed to a fall hazard of approximately 11 feet 6 inches when performing paint work from the bucket of a truck mounted aerial lift while not wearing a harness with a lanyard attached to the boom or basket of that lift.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work  shift to employees when they were in their work area(s):  On or about August 30, 2017, at the above addressed site, an employee was using chemicals including but not limited to PPG Paints 6-727XI Speed Hide Super Tech Interior Semi-Gloss paint while the employer did not maintain the safety data sheet for that product at the worksite.
