29 CFR 1910.135(a)(1): The employer shall ensure that each affected employee wears a protective helmet when working in areas where there is a potential for injury to the head from falling objects: a. On or before August 4, 2017, in the research building RB3 at Children's Hospital, the employer did not ensure that employees using a hoist to remove and reinstall a Weil 4-inch vertical wastewater pump in a pit with a manual Wesco HS type chain hoist wore hard hats to protect the head from falling objects. On or about August 4, 2017, an employee was injured and hospitalized during the lowering of the Weil 4-inch vertical wastewater pump into the pit when the anchorage point in the concrete ceiling failed causing all of the equipment to fall to the ground.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA: a. For a hospitalization injury involving an employee which occurred on or about August 4, 2017, in the research building RB3 at Children's Hospital, the employer did not report the hospitalization to OSHA within 24 hours.
