On or before August 4, 2017, in the research building RB3 at Children's Hospital, the employer did not ensure that employees using a hoist to remove and reinstall a Weil 4-inch vertical wastewater pump in a pit with a manual Wesco HS type chain hoist wore hard hats to protect the head from falling objects.  On or about August 4, 2017, an employee was injured and hospitalized during the lowering of the Weil 4-inch vertical wastewater pump into the pit when the anchorage point in the concrete ceiling failed causing all of the equipment to fall to the ground.
