29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):   On or about August 31, 2017, at the above addressed site, employees were exposed to a fall of approximately 14 feet 10 inches when performing stucco work on the work level platform of a fabricated frame scaffold that was not fully planked.

29 CFR 1926.451(b)(8): At all points of a scaffold where the platform changes direction, such as turning a corner, the platform that rests on a bearer at an angle other than a right angle was not laid first, and platforms which rest at right angles over the same bearer were not laid second, on top of the first platform:    On or about August 31, 2017, at the above addressed site, employees were exposed to a fall of approximately 14 feet 10 inches when performing stucco work on the work level platform of a fabricated frame scaffold that did not have planks installed between frame sections where the platform changes direction at a corner.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  On or about August 31, 2017, at the above addressed site, employees were exposed to a fall of approximately 14 feet 10 inches when gaining access to the work level platform of a fabricated frame scaffold that was not provided with a stair tower, ladder, ramp or direct access from another scaffold.

29 CFR 1926.451(g)(4)(xv): Crossbracing used in place of a midrail was not acceptable because the crossing point of two braces was not between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform or as a toprail when the crossing point of two braces was not between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform.  The endpoints at each upright were more than 48 inches (1.3 m) apart:  On or about August 31, 2017, at the above addressed site, employees were exposed to a fall of approximately 14 feet 10 inches when performing stucco work on the work level platform of a fabricated frame scaffold that had crossbracing being used as a top rail of a guardrail system and no midrail in place.
