29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Roof - On or about August 31, 2017, the employer did not ensure that employees were fall protected by guardrail systems with toeboards, safety net systems, or personal fall arrest systems while replacing the roof on a two story hotel, thus exposing employees to fall hazards up to about 20 feet, 9 inches onto concrete and/or earthen ground.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Lower Rear Roof - On or about August 31, 2017, employees used an aluminum extension ladder to gain access to the roof of the building without the side rails of the ladder being extended at least 3 feet above the upper landing surface, exposing employees to fall hazards of up to approximately 8 feet onto the first story roof overhang.
