The employer did not ensure that each affected employee uses appropriate body protection (leg protection) when exposed to hot water, and/or did not ensure that the PPE was worn.  Training on proper PPE was not conducted, on or about 09/01/17. 29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:   Three Taverns Craft Brewery, LLC -Decatur, GA - Chemical Mixing Area-  Employees were pumping and pouring corrosive cleaning chemicals, such as, but are not limited to Divosan Plus  (containing acetic acid, hydrogen peroxide, and peracetic acid),  Spectak G (containing 40%-50% sodium hydroxide), and Nonstick (containing 20-30% nitric acid and 5%-10% phosphoric acid) to clean tanks.  An emergency eye wash was not immediately available to flush both eyes simultaneously and the body in case of an emergency, on or about 09/01/17.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:   Three Taverns Craft Brewery, LLC, Decatur, GA - Employees were exposed to fall and struck-by hazards. The employer required employees to operate a platform which attached to the forks of the forklift to elevate employees to a higher working surface.  Employees were required to use the man basket approximately 18 feet into the air to introduce additives  to the top of the beer brewing vessels. The employer did not obtain prior written approval from the manufacture to modify and/or add to the forklift from it's original design, on or about 09/01/17.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
