An emergency eye wash was not immediately available to flush both eyes simultaneously and the body in case of an emergency, on or about 09/01/17. The employer required employees to operate a platform which attached to the forks of the forklift to elevate employees to a higher working surface. Employees were required to use the man basket approximately 18 feet into the air to introduce additives to the top of the beer brewing vessels. The employer did not obtain prior written approval from the manufacture to modify and/or add to the forklift from it's original design, on or about 09/01/17.
