29 CFR 1926.1427(f)(3)(iii): While monitoring the operator-in-training, the operator's trainer performed other tasks that detracted from the trainer's ability to monitor the operator-in-training.  in the alternative   29 CFR 1926.1427(f)(4): Continuous monitoring. The operator-in-training was not monitored by the operator's trainer at all times.  Site: 2306 3rd Avenue, New York, NY.  On or about 9/29/2017;  a) The operator's trainer was not monitoring the operator-in-training continuously during the rigging operation.  The hoisted load collided with a structural column, the load fell striking an employee of the rigging crew.
