29 CFR 1910.133(a)(5): The employer did not ensure that each affected employee used equipment with filter lenses that had a shade number appropriate for the work being performed, for protection from injurious light radiation. a. On or about August 29th, 2017, at the above addressed jobsite, in the warehouse, an employee was exposed to an eye injury hazard while using an oxyacetylene torch with clear safety glasses on.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. a. On or about August 29th, 2017, at the above addressed jobsite, in the warehouse, an employee was exposed to amputation hazards while working with a rotary tumbler that had a chain and sprocket guard missing.
