29 CFR 1910.119(f)(1)(iii)(B): Precautions necessary to prevent exposure, including engineering controls, administrative controls, and personal protective equipment;      The employer is failing to protect employees from fire and explosion hazards while engaged in petroleum refining activities.  This was most recently documented on or about September 4, 2017 at the HollyFrontier El Dorado Refining LLC, located at 1401 Douglas Road in El Dorado, Kansas.      a) Employers standard operating procedures for B-2503 did not include precautions necessary to prevent exposure, including administrative controls, indicating that B-2503 was not permitted to exceed the alarm set point for upper firebox temperature.    b) The operating practice of "bouncing the alarm" became a normal practice such that the furnace temperatures exceeded the upper firebox temperature (T125067) and the tube skin temperature (T125001) alarm limits on a regular/normal basis.

The documentation shall identify the date of the inspection or test, the name of the person who performed the inspection or test, the serial number or other identifier of the equipment on which the inspection or test was performed, a description of the inspection or test performed, and the results of the inspection or test.    Employer did not document each inspection and test that was performed on B-2503 during the course of the 2015 turnaround.
