29 CFR 1910.119(f)(1)(iii)(B): Precautions necessary to prevent exposure, including engineering controls, administrative controls, and personal protective equipment; The employer is failing to protect employees from fire and explosion hazards while engaged in petroleum refining activities. This was most recently documented on or about September 4, 2017 at the HollyFrontier El Dorado Refining LLC, located at 1401 Douglas Road in El Dorado, Kansas. a) Employers standard operating procedures for B-2503 did not include precautions necessary to prevent exposure, including administrative controls, indicating that B-2503 was not permitted to exceed the alarm set point for upper firebox temperature. b) The operating practice of "bouncing the alarm" became a normal practice such that the furnace temperatures exceeded the upper firebox temperature (T125067) and the tube skin temperature (T125001) alarm limits on a regular/normal basis.

29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information in paragraph 29 CFR 1910.119(d) before further use or in a safe and timely manner: The employer is failing to protect employees from fire and explosion hazards while engaged in petroleum refining activities. This was most recently documented on or about September 4, 2017 at the HollyFrontier El Dorado Refining LLC, located at 1401 Douglas Road in El Dorado, Kansas. The employer continued to operate the B-2503 furnace; a) at temperatures that exceeded the safe operating limits established for the heater's tube skin temperature when it was known that the furnace had a "considerable amount of creep damage" (i.e., deficient equipment). b) when the oxygen analyzer (A125322) was not functioning properly.
