(a) Zigzag Ranger District, North Compound Fire Warehouse, Zigzag, OR:  On or about August 30th, 2017 and at times prior thereto, the employer did not ensure that the upper tongue guards, on the Cummins 8" bench grinder, were installed properly.  This prevents the guards from being adjusted as the diameter of the wheel is reduced from usage.  Contact with an exploding grinding disc can cause severe lacerations and deep puncture wounds. (b) Barlow Ranger District, Clear Lake Lookout Fire Tower, Inside Tower, Dufur, OR:  On or about October 11th, 2017 and at times prior thereto, the employer did not ensure all unused openings on electrical boxes were closed. There were two (2) unused openings on the electrical junction box above the sink.  This exposed employees to an electrical shock of 110 volt, when operating the switch.
