29 CFR 1910.22(a)(1): Places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition: On or about September 9, 2017, the floor in and around the Cumberland grinder, west side of grind room, was strewn with plastic debris, which could lead to trips and falls and also a potential fire hazard.

29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by way of guardrail system, safety net, or personal fall arrest: On or about September 6, 2017, the employer had not ensured that all sides of the platform for the east side grinder in the grind room had proper fall protection in place. The back edge of the platform was approximately 67 inches (5 feet 7 inches) above floor level with no fall protection in place.

29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized: a. On or about October 4, 2017, in the grind room, the operator was exposed to continuous noise at 339% of the permissible daily exposure (8-hour time-weighted average sound level of 90 dBA) or equivalent sound level of approximately 99.4dBA during the 440 minute sampling period. Exposure calculations include a zero increment for the 40 minutes not sampled. Abatement Steps: 1. Effective hearing protection shall be provided and used by all exposed employees as an interim protection measure and when administrative or engineering controls fail to reduce sound level within the levels of table G-16. In addition, an effective hearing conservation program in accordance with 29 CFR 1910.95(c) thru (n) shall be maintained. ABATEMENT MUST BE COMPLETED BY (15 days after issue date). 2. Submit to the Area Director a written detailed plan of abatement outlining a schedule for the implementation of engineering and/or administrative measures to control employee exposure to noise as referenced in this citation. This plan shall include at a minimum, target dates for the following actions which must be consistent with the dates required by this citation. ABATEMENT MUST BE COMPLETED BY (45 days after issue date). a) Evaluation of engineering control options. b) Selection of optimum control method and completion of design. c) Procurement, installation and operation of selected control measures. d) Testing and acceptance or modifications/redesign of controls. 3. Abatement shall have been completed by the implementation of feasible engineering and/or administrative controls upon verification of their effectiveness in achieving compliance. ABATEMENT MUST BE COMPLETED BY (60 days after issue date).

29 CFR 1910.95(c)(1): The employer had not administered a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section, whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response) or, equivalently, a dose of fifty percent: a. On or about October 4, 2017, for the inline #10 operator who was exposed to continuous noise levels at 113.6% of the action level (8-hour time-weighted average sound level of 85 dBA) during the 444 minute sampling period. Exposure calculations include a zero increment for the 36 minutes not sampled. b. On or about October 4, 2017, for the grind room operator mentioned in citation 1, item 3(a).
