29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:    (a)  The Northview Group LLC on or about September 06, 2017, at the address of 1225 W. Alexis Rd. Toledo, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of 29 CFR 1926.761:  a) The Northview Group LLC on or about September 06, 2017, at the address of 1225 W. Alexis Rd. Toledo, OH: The employer did not ensure that employees had been trained in the recognition of fall hazards commonly created during steel erection activities. Employees unfamiliar with the requirements of fall protection during steel erection activities were exposed to falls up to 18 ft.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    a)  The Northview Group LLC on or about September 06, 2017, at the address of 1225 W. Alexis Rd. Toledo, OH: The employer did not ensure employees were adequately protected from falls during steel erection activities. Employees installing steel siding from atop a steel framed structure without the benefit of fall protection were exposed to falls exceeding 18 ft. to the ground below.
