Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems.  On 8/17/2017, employees from Michael Bazan were removing roofing materials from a roof of a residential building located at 2060 Hannah Drive in Wentzville, MO. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.   The crew leader and three employees were observed on a 4/12 and 10/12 sloped roof approximately nine feet above the ground without fall protection, exposing them to a fall to the ground below.

On 8/17/2017, employees from Michael Bazan were removing roofing materials from a roof of a residence located at 2060 Hannah Drive in Wentzville, MO.  The ladder the employees were using to access the roof did not extend three feet beyond the upper landing surface or edge of the roof.
