29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) 244 W Beach Blvd, Long Beach, Mississippi; On or about September 7, 2017; The employer did not ensure an employee working from a Genie aerial lift(RE 4355) was protected by a personal fall arrest system which exposed the employee to fall hazards up to 18 feet to the ground below.  b) 244 W Beach Blvd, Long Beach, Mississippi; On or about September 7, 2017; The employer did not ensure an employee working from a Bil-Jax aerial lift was protected by a personal fall arrest system which exposed the employee to fall hazards up to 18 feet to the ground below.

29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question:  a) 244 W Beach Blvd, Long Beach, Mississippi; Aerial lift training: The employer failed to ensure an employee working from an aerial lift was training on the hazards associated with that work which would include fall protection exposing the employee to potential falls of up to 18 feet.
