29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be nescessary to comply with the standards set forth in 29 CFR 1926 as evidenced by the violation(s) noted during the course of the inspection:  a)  James Home Improvement on or about September 12, 2017 at the address of 514 Main Street, Huron, Ohio: The employer failed to ensure an accident prevention program was in place to aid the employer and his employees in the recognition and avoidance of hazards associated with residential roofing operations (i.e.; fall protection, and use of personal protective equipment).

29 CFR 1926.503(a)(1): A training program was not provided to each employee who might be exposed to fall hazards, to enable that employee to recognize the hazards of falling, and be trained in the procedures to be followed to minimize these hazards:  a) James Home Improvements on or about September 12, 2017 at the address of  514 Main Street, Huron, Ohio: The employer had not trained employees in the appropriate use of fall protection, such as but not limited to, the use of personal fall arrest systems while performing various roofing operations.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems:   a)  James Home Improvement on September 12, 2017 at the address of 514 Main Street, Huron, Ohio: The employer did not ensure employees were protected from falls while installing new roofing material to the residential type structure.  The employees were exposed to a potential  22 foot fall to the level below.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet above the upper landing surface being accessed, and the ladder was secured at its top to a rigid support that will not deflect, a grasping device was not provided to assist employees in mounting and dismounting the ladder.   a) James Home Improvement on or about September 12, 2017 at the address of 514 Main Street, Huron, Ohio, 44839: The employer did not ensure the side rails of the 24 feet fiberglass extension ladder was extended at least three feet above the landing surface. One employee was exposed to falls while using the ladder to access the roof.
