29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   On or about September 12, 2017 at a worksite located at 2487 West 7th Street, Cleveland, OH 44113, during construction activities on a commercial roof, employees were exposed to fall heights of approximately 25 feet. The employer did not ensure employees were protected from fall hazards by ensuring a fall protection system/device was being used.
