29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  a)  Jobsite located at 65 Harvest Ridge Road, Howell NJ:   Company employees from Superior WS Covering LLC., were exposed to fall hazards of approximately 14 feet to the ground surface below when using two ladder pump jack scaffolds that did not have guardrails installed along the open ends of the platforms while installing the siding on the residence under construction.  Occurred on or about 08/28/2017.

29 CFR 1926.451(g)(4)(v): Screens and mesh were not extended from the top edge of the guardrail system to the scaffold platform, and along the entire opening between the supports.  a)  Jobsite located at 65 Harvest Ridge Road, Howell NJ:   Company employees from Superior WS Covering LLC., were exposed to fall hazards of approximately 14 feet to the ground surface below when using two ladder pump jack scaffolds that have a missing part of the midrail or mesh while installing the siding on the residence under construction.  Occurred on or about 08/28/2017.
