29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports:  On or about 9/5/2017 at 3550 Sweetwater Road, Building 200, Lawrenceville, GA. An employee was observed exposed to fall hazards of 14-feet to the lower level while performing stucco and siding removal, installation, and repair work from a scaffold that was not fully decked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  On or about 9/5/2017 at 3550 Sweetwater Road, Building 200, Lawrenceville, GA. workers performing siding and stucco removal, repair, and replacement work were exposed to fall hazards:  a. where a worker was observed exposed to 14-foot fall hazards when climbing down the frame; and   b. where access to the levels above the 3rd platform required using the cross bracing or frames where no ladder was provided exposing workers to potential 30-foot fall hazards.

On or about 9/5/2017 at 3550 Sweetwater Road, Building 200, Lawrenceville, GA. workers performing siding and stucco removal, replacement and repair work were exposed to fall hazards above 30-feet to the ground where the employer  failed to install guard rails.
