29 CFR 1910.178(m)(5)(i): When a powered industrial truck is left unattended, load engaging means were not fully lowered, controls were not neutralized, power was not shut off, and brakes set    On or about September 06, 2017, at the worksite located 3901 Union Boulevard, Suites 135 & 235, Saint Louis, Missouri the Clark Propane Forklift (Model:  CGC25; Serial Number: C365L-0257-9488FB) was found unattended with the load engaging means elevated to approximately 23 inches from the floor.

The Clark Propane Forklift (Model:  CGC25; Serial Number: C365L-0257-9488FB) was found with following defect(s):    1.  Broken propane tank clip broken leaving tank unsecure 2. One headlight missing (broken off completely)
