29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) John Wright CustomChrome Plating on or about September 13, 2017 the employer did not ensure that all one or more methods of machine guarding was provided to protect employees using the belt sander. Employees working in the polishing and grinding area with the OHAS E. Commodious and Sons Company belt sander were exposed to caught by hazard from unguarded ingoing nip points on the front and back rollers.    b) John Wright CustomChrome Plating on or about September 13, 2017 the employer did not ensure that all one or more methods of machine guarding was provided to protect employees using the belt sander. Employees working in the polishing and grinding area with the Gardner belt sander, serial number 582390, were exposed to caught by hazard from unguarded ingoing nip points on the front and back rollers.

Employees working in the polishing and grinding area were exposed to caught by hazards from 3 inches on the right and 3.5 inches on the left of unguarded horizontal shafting approximately 4 feet from the floor while working on, but not limited to, machine #10 Queen City SN: 206337.     b) John Wright CustomChrome Plating on or about September 13, 2017 the employer did not ensure that all exposed parts of horizontal shafting seven feet or less from the floor were completely guarded. Employees working in the polishing and grinding area were exposed to caught by hazards from 8 inches on the right and 9 inches on the left of from unguarded horizontal shafting approximately 4 feet from the floor while working on, but not limited to, machine #9 the BalDor Buffer.     c) John Wright CustomChrome Plating on or about September 13, 2017 the employer did not ensure that all exposed parts of horizontal shafting seven feet or less from the floor were completely guarded. Employees working in the polishing and grinding area were exposed to caught by hazards from 8 inches on the right and 7 inches on the left from unguarded horizontal shafting approximately 4 feet from the floor while working on, but not limited to, machine #8 the Hammond buffer.

Employees working with, but not limited to, the BalDor buffer (#9) in the polishing and grinding area were exposed to caught by hazards from unguarded shaft ends that projected 2 inches on the left with a shaft diameter of 1 inch and 2 inches on the right with a shaft diameter of 1 inch.     b) John Wright CustomChrome Plating on or about September 13, 2017 the employer did not ensure that all  unguarded projecting shaft ends did not project more than one half the diameter of the shaft. Employees working with, but not limited to, the Hammond buffer (#8) in the polishing and grinding area were exposed to caught by hazards from unguarded shaft ends that projected 1.5 inches on the left with a shaft diameter of 2 inches and 1.5 inches on the right with a shaft diameter of 2 inches.

Industrial hygiene wipe sampling showed lead contamination of the lunch table at 285.73 micrograms per 100 centimeters squared. Exposure to lead can cause permanent damage to the kidneys and nervous system.

Industrial hygiene wipe sampling showed cadmium contamination of work table at 23.207 micrograms per 100 centimeters squared. Exposure to cadmium can cause permanent damage to the kidneys and lungs.
