29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:    a.   On or about September 6, 2017, on the 6:12 pitch roof and trusses, employees installing OSB sheathing and truss framing were exposed to fall hazards of 10 feet to 15 feet high from the eaves and down through the trusses, in that they were not protected by a fall protection system.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:     a.  On or about September 6, 2017, at the left side of the house, a folded step ladder was leaned against the outside wall and used by an employee as a non-self-supporting ladder, a purpose for which the manufacturer did not design the ladder to serve.
