29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):  a. On or about 09/01/2017, at the job site - employees were exposed to struck-by hazards, in that, the forklift was not removed from service after the horn was found defective and documented in several operators daily check list.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  a. On or about 09/01/2017, at the job site - the connection plug of a forklift battery charger was partially separated from the flexible cable, in that, the insulation casing was not covering completely and bare wire was visible.
