29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    a) South Mill Street West Roof - Portable ladder set 23 feet high did not extend 3 feet about the upper roof and was not secured to a rigid support, thereby exposing workers to falls of approximately 10- 23 feet to the ground below on or before September 19, 2017.      b) South Mill Street North Roof - Portable ladder set 10 feet high did not extend 3 feet about the upper roof and was not secured to a rigid support, thereby exposing workers to falls of approximately 10 feet to the ground below on or before September 19, 2017.      No abatement certification or documentation required.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    a) South Mill Street - The employer did not ensure that employees engaged in roofing work approximately 10 to 23 feet above the lower levels were protected from heights by guardrail systems, safety net system, or personal fall arrest system, thereby exposing employees to falling hazards on or before September 19, 2017.    No abatement certification or documentation required.
