29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling:    a) On or about September 17, 2017, Chisholm Trail Terminal failed to ensure that employees of a contractor, Central States Fumigation and Services, were protected from the hazard of falling while performing fumigation services at the Terminals rail shed.      b) On or about September 17, 2017, Chisholm Trail Terminal failed to ensure that employees of a contractor, Central States Fumigation and Services, were protected from the hazard of falling while performing fumigation services for an international train.

On September 17, 2017, Chisholm Trail Terminal, failed to inform contractors of the applicable safety rules of the facility for employees walking/working on railcars performing fumigation exposing employees to hazards such as falls from heights.
