29 CFR 1910.134(c)(1):  This standard is applicable to construction work under this section:  29 CFR 1926.103; A written respiratory protection program that included the provisions in 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use::   The employer is failing to protect employees from respiratory hazards associated with spraying paint without respiratory protection program.  The most recent occurrence of this was evident during the inspection of the job site located at Building 563, Offutt AFB, NE where the employer has employees spraying Sherwin Williams Direct-To-Metal Alkyd Enaml on the ceiling inside the building while wearing half mask respirators without having conducted medical evaluations and fit testing.

29 CFR 1926.451(g)(1):  Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   The employer is failing to protect employees from fall hazards associated with working off of scaffolding while conducting overhead work.  The most recent occurrence of this was evident during the inspection of the job site located at Building 563, Offutt AFB, NE where the employer has employees exposed to fall hazards while painting off of a double stack of frame scaffolding without guard railing or other conventional fall protection methods.
